WE ARE ACCEPTING NEW PATIENTS AGE 12 AND UNDER!
If you are interested in becoming a patient of our practice please complete Child Form below, one for each child.
If you are an Adult and would like to be added to our new patient wait list, please fill out the Adult Form and we will reach out when the practice has availability - thank you for your patience and understanding.
Please note, it may take a few days for us to reach out to you. If you haven't heard from us in two weeks, feel free to call us. We just ask you wait a few weeks before reaching out as we work through the incoming requests.

CANCELLATION/NO SHOW POLICY AT AFP
If you must cancel your appointment please note the following policy: All cancellations require a 48-hour notice. If you do not cancel within that timeframe you will be charged a $50 no show/late fee. Please DO NOT leave a voicemail over the weekend. This will not be accepted as a 48-hour notice. You need to call or email the office on the Friday before. If you know you are going to be late, please call to let us know. We will do everything we can to try to accommodate you.

Deb Ellis, the Physical Therapist, is amazing. She cared for my needs as a dental Hygienist when I was experiencing neck and lower back issues.
